CITY OF ALBANY CIVIL SERVICE APPLICATION INSTRUCTIONS

GENERAL INFORMATION TAB

SECTION A – NAME AND MAILING ADDRESS:

Indicate your name mailing address for correspondence. We will make reasonable effort to mail all correspondence to the MOST RECENT mailing address you have provided in connection with a current exam process.

SECTION B – RESIDENCE:

Information in this section is required for all applicants. Complete the requested information regarding your legal domicile. This section MUST include a physical street address (i.e. 14 State Route 2345), and CANNOT include a Post Office box. While you may receive mail at more than one address, you can be a LEGAL RESIDENT OF ONLY ONE DOMICILE at any given lime.

SECTION C – VETERAN’S INFORMATION: (EXAMS ONLY)
  • Honorably discharged veterans of war periods may apply for additional points on civil service examinations administered by the City of Albany Civil Service Commission. Both the Application for Veteran's Credits and the Application for Veteran’s Disability Records Release, can be obtained from the Civil Service Office. Follow the instructions on the Application for Veteran's Disability Records Release form carefully. In order to receive veterans credits, the completed forms must be received by the Civil Service Office before the eligible list for the examination is established.

SECTION D – CHILD OF FIREFIGHTER OR POLICE OFFICER: (EXAMS ONLY)

Answer accordingly.

SECTION E – AGE/DATE OF BIRTH & CITIZENSHIP:
  • Answer “Yes” if you are UNDER the age of 18 and provide your date of birth and answer following question if you possess the appropriate Student General Employment Certificate.

  • If you are applying for Police Officer or Firefighter please provide your date of birth.

  • Answer whether you are a Citizen of the United States or not. If you are not a U.S. Citizen please answer if you have the legal right to accept employment in the United State.

SECTION F – SPECIAL ACCOMMODATIONS IN TESTING & ALTERNATE TEST DATE: (EXAMS ONLY)

TESTING ACCOMMODATIONS: Accommodations in testing will be provided for individuals with disabilities. Persons who are requesting testing accommodations must make a request in writing, to the City of Albany Civil Service Office no later than 14 days prior to the exam date, clearly stating the type of accommodation requested and the reason for the request. If your request and/or documentation are not received within the requested time limits, your request will not be able to be granted.

ALTERNATE TEST DATE: Candidates are expected to make adjustments in their personal commitments in order to appear at the announced examination site on the scheduled test date. An alternate test date may be approved if there are compelling circumstances resulting from an emergency or a conflict beyond the candidate's control between the scheduled test date and an event of serious importance. The Civil Service Office will consider legitimate circumstances relating to individual requests for an alternate test date. Requests for an alternate test date must be submitted in writing using the Alternate Test Date Application available in the online application. If you require an alternate test date, please complete this form and attach it to the “Attachments” tab of your application for the exam in which you are requiring an alternate test date for.

SECTION G – DISCIPLINARY REMOVAL HISTORY:

SECTION H – CROSSFILER INFORMATION: (EXAMS ONLY)

Candidates who have applied with other municipalities (including the State of New York) to take civil service examinations to be held on the same date as the City of Albany’s examination must complete and return this form at least 14 days prior to the date of the examination. Candidates must make arrangements to take all the examinations at one test site. Candidates must notify all local government Civil Service agencies with which they have filed an application of the site where they wish to take their examinations.

This should not be used if the candidate is taking multiple examinations administered by the City of Albany Civil Service.

If you have applied for both State and Local Government examinations, you must make arrangements to take all of your examinations at the STATE examination center by calling (518) 474-6470 no later than 14 days prior to the test date.

Complete as requested. Do not mail a copy of your transcript unless requested by the exam announcement.

Complete as requested. It is NOT necessary to note other classes that you hold if they are not listed as a requirement for the job that you are applying for.

SECTION L – PROFESSIONAL LICENSES & CERTIFICATION:

Complete as requested.

SECTION M – EXPERIENCE:

Start with your most recent experience. List all employment, military service, or volunteer experience that proves you meet the minimum qualifications for the job or exam for which you are applying. We cannot interpret omissions or vagueness in your favor. You are responsible for an accurate and clear description of your experience.

For DUTIES describe the nature of the work which you personally performed including the estimated percentage of time spent on each type of activity. If you supervised, state how many people and the nature of such supervision.

NOTE: Applicants may be conditionally admitted to an exam on the basis of statements made on the application. Such statements may not be reviewed and/or verified until after the exam is held. At that time those candidates not meeting the requirements are disqualified. Candidates who are subsequently disqualified after taking the test will NOT be notified of their score.
